Informed Consent & Telehealth Disclosure

Psychotherapy services may be provided in person or through secure video-based telehealth platforms (e.g., Simple Practice or Doxy.me). Before beginning treatment, clients will be asked to review and sign a full informed consent form, which includes acknowledgment of the following:

  • Nature of Telehealth Services: Telehealth involves the use of electronic communications to support long-distance clinical care. It can be a convenient and effective way to access mental health services, particularly for clients in remote locations or with mobility concerns.

  • Differences from In-Person Services: Telehealth may not be appropriate for all clients or clinical issues. There may be situations where in-person care is recommended to ensure effective treatment.

  • Technology & Privacy Risks: While we use HIPAA-compliant platforms and encrypted systems, telehealth carries potential risks, such as technical failures or unauthorized access. Clients are encouraged to use secure, private internet connections and to avoid public Wi-Fi.

  • Client Rights: You have the right to ask questions about telehealth, decline or withdraw consent at any time, and request in-person services if clinically appropriate and available.

  • Emergency Situations: Telehealth is not suitable for emergency situations or clients in acute crisis. If you are experiencing a mental health emergency, call 911 or go to the nearest emergency room.
